Support At Home

Momentum Care offers flexible and personalised support at home packages to assist people to live independently in their own homes and to have control over their daily lives.

Support is flexible and can range from a few hours a week to 24-hour support. Working with the person concerned, their family and professional agencies, we develop a person-centred plan tailored to meet each person's needs and wants.

Our services assist people with:

  • getting up/going to bed
  • meal preparation
  • visiting family and friends
  • shopping
  • attending social events and appointments
  • housework

  • Who is it for?

    Momentum Care’s support at home services support adults with wide-ranging needs, including older people, people with physical and learning disabilities, people with an acquired brain injury, traumatic brain injury survivors, people with end-of-life support needs and others who require support on a short or longterm basis.

  • How can it be accessed?

    People can contact Momentum Care direct for details of the service that is nearest to them. Our staff will be able to explain what options are available in terms of availing of the service together with the steps that can be taken in sourcing access to services e.g. through social services.

    Anyone can make contact with their respective local authority with regards to having their needs assessed in determining what support at home may be required.

  • How much does it cost?

    Everyone’s needs are different and because our support packages are tailored to meet each person's needs, we calculate costs on an individual basis.

    To make sure we give an accurate cost, we will meet with the individual and their family to discuss needs. This will make it easier to advise on the costs arising.
